چکیده
case presentation a 43-year-old man was referred to our institution for a severe aortic regurgitation and an ascending aorta enlargement associated with a relapsing sub-aortic membrane, which had been treated in his childhood. he underwent a redo ascending aorta replacement and an aortic valve replacement (mechanical prosthesis) along with a resection of the relapsing sub-valvular membrane. some weeks after he was admitted to another hospital for fever, arthromyalgias and chest pain. after a few days, he developed an inflammatory jugular swelling and underwent a chest ct scan showing a bulky anterior mediastinal abscess in the direct continuity with the sternum. a further chest ct scan showed a mediastinal para-aortic capsulated mass, 135 × 85 × 90 mm in dimension, well delimited over the surrounding plans. at the top of the lesion, a further fluid collection was appreciated, extending through the sternum over the subcutaneous tissues, 30 × 20 mm in dimension. re-sternotomy was performed and the mediastinal mass was opened and drained. after an extension of the median sternotomy through the epigastrium, a pedunculated tract of the greater omentum was mobilized and positioned around the vascular prosthesis to fill the empty space left by the abscess. introduction a 43-year-old man developed a mediastinal abscess after a redo aortic valve along with an ascending aorta replacement and also a sub-aortic membrane resection. he was surgically revised: as there were no signs of involvement of the valve and vascular prosthesis, a pedunculated tract of the greater omentum was mobilized and positioned around the aortic prosthesis. during a 10-month follow-up, it has been revealed that the patient is doing well and has no recurrence of the infection. use of the great omentum could be considered in the selected mediastinitis cases. conclusions the greater omentum is well known in cardiothoracic surgery for its valuable features such as plasticity, immune competence, good blood supply and neovascularization-potential. dead space can be obliterated by omental flap because of its plasticity qualities. it contains a large number of immunologically active cells likely to be responsible for its anti-infective properties. we can affirm that use of the great omentum could be considered in selected post-operative mediastinitis cases after cardiac surgery.

Treatment of severe mediastinitis following cardiac surgery with omental flaps.
1 of 2 DESCRIPTION The incidence of mediastinal wound infection in patients undergoing open heart surgery can be up to 4%. 1 A subgroup of 20–30% of those patients as per El Oakley and Wright 2 develops deep sternal infections. Radical debridement is of a paramount importance, therefore sternal excision becomes a necessity in cases with severe sternal osteomyelitis. Under those circumstances va...
متن کاملPost-sternotomy mediastinitis after cardiac surgery
Post-sternotomy mediastinitis (PSM) is a rare but devastating complication, which may occur after cardiac surgery, and which increases mortality and morbidity tremendously. This study showed a statistically significant increased in-hospital mortality of patients with deep sternal wound infection (DSWI) compared with patients without DSWI (8.1% versus 2.7%, respectively) (p=0.03). For long-term ...

Figure 1. Mediastinal defect with exposed pros-thetic aortic root graft. DESCRIPTION A 70-year-old woman with short stature and hypermastia underwent ascending aortic aneurysm repair with a prosthetic graft, complicated by postoperative mediastinitis and sternal dehiscence. She was treated with aggressive sternal debridement and negative pressure wound therapy for 1 week (Fig 1) prior to defini...

INTRODUCTION — Prior to the development of modern cardiovascular surgery, most cases of mediastinitis arose from either esophageal perforation or from contiguous spread of odontogenic or retropharyngeal infections. Rarely, primary infections of the mediastinum developed as a result of penetrating trauma or hematogenous spread of infection. However, in modern practice, most cases of mediastiniti...
متن کاملDaptomycin as supportive treatment option in patients developing mediastinitis after open cardiac surgery
BACKGROUND Mediastinitis is a severe complication after cardiac surgery. While improvement of prophylaxis and of medical and surgical therapy has reduced its incidence, the treatment of mediastinitis continues to be a challenging problem. Within this study, we report the successful use of daptomycin as supportive therapy in patients developing mediastinitis after open cardiac surgery. METHODS...
متن کاملTroponin-I release after cardiac surgery with different surgical techniques and post-operative neurological outcomes
Cerebral hypoperfusion during cardiopulmonary bypass surgery has been thought to be a factor in the aetiology of brain damage with evidence of post-operative neurological deficits. Cardiac-specific biomarkers such as troponin-I, troponin-T and CK-MB have been used extensively to predict myocardial injury and ischaemia. This prospective study investigated the level of troponin-I release in both ...
